This form is for anonymous reporting to the Title IX Office only. If you are a non-confidential employee, you must provide the name of the Complainant (victim/victim-survivor) of alleged prohibited conduct and file a report using the following Reporting Link.

If you intend on filing an anonymous report, please know that the Title IX Office will likely be limited in providing resources, support, and options to you. A list of resources, including Confidential Resources, is available on the GMU DEI Website. Anonymous Reports are documented by the Title IX Office and these records are maintained. If you are the victim-survivor of prohibited conduct, you may also file a report via the above reporting link--if you do this, the Title IX Office will contact you to see if you would like to meet to discuss resources, supportive measures, and options. 

The University will make reasonable efforts to evaluate the report promptly and act accordingly, however again, the University will likely be limited in taking any action on anonymous reports. Under the Clery Act, the University is required to provide certain crime statistics and reports of sexual violence to the Mason University Police Department and local law enforcement. If you or someone you know would like to speak to a confidential resource for support, you may also contact one of the following departments: Student Support and Advocacy Center (SSAC); Counseling and Psychological Services (CAPS);  Student Health Services (SHS); and/or the University Ombudsperson.  Faculty and staff seeking a confidential resource can contact the Employee Assistance Program.
